Mandated Reporters & SOC-341 Form
Mandated reporters must report any abuse they know of, suspect, or have reason to believe is happening, including physical abuse, abandonment, isolation, financial abuse, neglect, and self-neglect. The SOC 341 form, required by the California Department of Social Services under WIC Sections 15630 and 15658(a)(1), documents information on the abuse or neglect of elderly or dependent adults.

Ombudsman Posters
Each skilled nursing facility and assisted living facility must display copies of the Ombudsman posters as required by Section 9718 of the Welfare and Institutions Code. To obtain posters, please contact our program at 661-323-7884.
Nursing Facility Discharge Guidelines: Nursing facilities must provide written notice to the resident and their representative in a language they understand before transferring or discharging a resident. A copy of the notice must also be sent to the long-term care ombudsman program.
